A Senior Solutions Care Manager can be a valuable ally as one works through health care and other issues for elders.  A Care Manager can improve one’s ability to care for a dependent relative and maximize the capacity of the older person to make informed and appropriate decisions.  A Care Manager will:

• Maintain the great amount of independence and human dignity for your elder

• Enable the person to remain in the most appropriate environment

• Provide an appropriate, comprehensive and coordinated response to the person’s needs

• Address prevention as well as rehabilitation and maintenance

• Increase one’s access to community-based services and quality providers

• Build and strengthen family and community support

• Identify and counsel both you and your elder in times of depression and through difficult transitions

• Support you throughout the aging process
