Diabetic Management
The care of older adults with diabetes can be more complicated and often need the assistance of a clinical person to support and assist in the management of their diabetes. A St. Andrew’s Care Manager can provide this assistance.

Provide the oversight necessary to ensure compliance with medical regiment.

Monitor condition and other complications of chronic conditions.

Support self management and provide support only where needed.

Ensure goals are made and progress monitored.

Provide the vital education necessary for understanding the disease and the treatment.

Serve as a liaison between the senior and the medical providers.
